Retinoids are among the few topical ingredients with substantial evidence behind them, and they are also among the slowest to show results. The delay follows directly from how they work.

They act inside the cell nucleus

Retinoids are derivatives of vitamin A, and once inside a skin cell they bind receptors that sit on the DNA itself.

Those bound receptors alter which genes are transcribed, changing the proteins a cell produces rather than acting directly on any structure already present.

This is a fundamentally different mode of action from moisturisers or acids, which alter the surface directly and therefore show effects far sooner.

Conversion adds further steps

Only one form binds the receptor directly. The forms sold without prescription must be converted within the skin through one or two enzymatic steps first.

Each conversion is incomplete, so a weaker precursor delivers considerably less active compound than its concentration suggests.

This is why prescription strength differs from cosmetic strength in kind as well as in degree, and why the two are not comparable by percentage on a label.

The tissue itself changes slowly

Altered gene expression changes the behaviour of cells, but the visible layer must then be replaced by cells produced under the new instructions.

Turnover of the surface layer takes weeks, and collagen deposition in the dermis below proceeds over months, so structural change accumulates gradually.

Studies of these ingredients generally run for extended periods for this reason, and effects assessed at a few weeks understate what continued use produces.

Irritation arrives before benefit

Early use commonly produces redness, flaking and sensitivity as turnover accelerates and the barrier is temporarily disrupted.

This phase typically settles as the skin adjusts, which is why gradual introduction and reduced frequency at the start are standard advice.

The sequencing is unhelpful for adherence, since the unpleasant part comes first and the benefit arrives long after most people would have judged the product.

Sun sensitivity and supervision

Retinoid-treated skin is more sensitive to ultraviolet radiation, and several of these compounds degrade in light, which is why evening application is standard.

Daily sun protection is treated as part of the regimen rather than an optional addition, since ultraviolet damage works against the same collagen the treatment is building.

These compounds are not appropriate during pregnancy, and stronger forms require a prescription, so use should be set out by a doctor or dermatologist.
